C.B. Boswell, MD, FACS, came to St. Louis in 1995 to complete his residencies in General Surgery and Plastic Surgery at Washington University’s Barnes-Jewish Hospital. Part of this training was with Dr. Leroy Young, who was then a Professor in the Department of Plastic and Reconstructive Surgery. In his final year at Washington University, Dr. Boswell was chosen as Executive Chief Resident in Plastic Surgery. He earned his Board Certification from the American Board of Plastic Surgery in 2003 and recertified in 2013. He is a Fellow of the American College of Surgeons (FACS) and is a member of the American Society of Plastic Surgeons (ASPS) and the American Society for Aesthetic Plastic Surgery (ASAPS). He has been president of St. Louis Area Plastic Surgery Society and president of the Missouri Association of Plastic and Reconstructive Surgeons. Cosmetic surgery can re-contour just about every area of the body. For example, liposuction, which removes excess fat deposits, can be performed on the neck, upper arms, waist, abdomen, hips, flanks, buttocks, thighs, knees, calves, and ankles. Liposuction is increasingly requested by men who cannot get rid of “love handles” no matter how much they exercise. For many women, no amount of sit-ups will flatten the abdomen after childbirth. In these cases, liposuction combined with a “tummy tuck” (either a standard or mini-abdominoplasty) usually solve the problem. Body contouring does not always involve fat removal. Some patients have sagging or excess skin that needs to be removed. Skin sagging is typically caused by aging, while skin excess may result after a major weight loss or liposuction. Re-contouring lifts of the arms (brachioplasty), breasts, upper body (torsoplasty), thighs, buttocks, or entire lower body (called a circumferential body lift) are specialties at our practice. The art of plastic surgery plays a major role in all body contouring procedures because the surgeon basically sculpts the body into a pleasing contour. Most women have some degree of cellulite by the time they are 35, even those who are not overweight. As we age, gravity disrupts the tight bonds between skin and its underlying supportive tissues, and fat cells grow into the lower layers of the skin. This process creates the dimpled appearance known as cellulite. In women of normal weight, cellulite can worsen because of skin and tissue laxity. What is Liposuction? BodyAesthetic Plastic Surgery & Skin Care Center offers St. Louis plastic surgery patients liposuction to achieve a smoother, sleeker figure. In the paragraphs below, the liposuction procedure is described. Liposuction sculpts the body by removing unwanted deposits of fat and can be done on nearly any area of the body, including the abdomen, hips, flanks, back, buttocks, thighs, knees, ankles, upper arms, chin, cheeks and neck. It is the most popular plastic surgery procedure in the U.S., performed on nearly half a million Americans in 2004. Increasingly requested by both men and women, liposuction is ideal for getting rid of localized areas of fat that will not go away no matter what you do. During the past decade, liposuction (also known as “lipoplasty” or “suction lipectomy”) has benefited from several new refinements and techniques that produce more precise results and quicker recovery times. Although liposuction is not a substitute for losing weight, it can remove stubborn areas of fat that don’t respond to traditional dieting or exercise. Am I a Candidate for Liposuction in St. Louis? The best candidates for liposuction are people of normal weight with firm, elastic skin who have pockets of excess fat in certain areas. They should be physically healthy, psychologically stable and have realistic expectations. Although age is not a major consideration, older patients tend to have skin that is less elastic and therefore may not get the same results as a younger patient with tighter skin. Liposuction carries greater risk for individuals with medical problems such as diabetes, obesity, significant heart or lung disease, poor blood circulation, tobacco use, or those who have recently had surgery near the area to be contoured. Your surgeon will determine whether special precautions need to be taken for liposuction to be safe for you. In your initial consultation, your surgeon will evaluate your health, determine where your fat deposits lie, assess the condition of your skin, and explain the body contouring method most appropriate for you. Be honest in discussing your expectations with your surgeon, and he will be equally frank when explaining the risks and limitations of treatment. Liposuction is performed through small incisions, less than a half-inch in length, that are made in an inconspicuous location near the area to be treated. The first step is to inject (infuse) a specified amount of fluids that is roughly equal to the amount of fat to be removed. This fluid consists of saline (salt-water) solution that contains epinephrine. The saline plumps up the fatty tissue, and the epinephrine constricts the blood vessels to minimize bleeding. A slim hollow tube, called a cannula, is inserted through the incision and moved back and forth through the tissue to break up the fat cells, which are then suctioned out, along with the infused fluids. If multiple body areas are treated, your surgeon moves from one area to the next repeating the steps of infusing fluids, breaking up the fatty tissue, and sucking it out. The preceding paragraph briefly describes what is called suction-assisted lipoplasty (SAL), which is the standard type of liposuction used in St. Louis by Dr. Boswell. In certain situations, he also may use ultrasound-assisted lipoplasty (UAL), which has a special cannula that produces ultrasonic energy. This ultrasonic energy explodes the walls of fat cells to liquefy the fat, which is then suctioned out. UAL improves the effectiveness of liposuction in fibrous areas of the body, such as the upper back or the male abdomen, and is also effective for patients undergoing a second liposuction in an area previously treated. In general, UAL takes longer to perform than SAL. The time required to perform liposuction varies considerably, depending on the number and sizes of areas being treated. As Dr. Boswell works, he basically sculpts the body contour with differently-sized cannulas and pays special attention to making sure both sides of the body are as symmetrical as possible. Because fluids are infused into the tissues and then suctioned out with the fat, careful monitoring of patients’ fluid levels is essential to make sure their fluid balance remains within safe limits. Being overloaded with fluids or having fluid volume depleted can both lead to serious complications if fluids are not managed properly. Most patients need to receive intravenous fluids during and immediately after surgery.

A “Mommy Makeover” is designed to address the changes of a woman’s body that have occurred as a result of pregnancy. This can include changes to the face, breasts, trunk, and extremities. There is not specific procedure or group of procedures that will address all of the issues caused by pregnancy. Every person will have different changes that they see during pregnancy, so the treatment options will vary according to your specific issues. Mommy Makeover Facial Treatments One of the more common facial issues is hyperpigmentation of the facial skin (malar mask of pregnancy) due to estrogen fluctuations during pregnancy. Typically, the first step to address skin issues is to start a good skin care regimen with physician strength skin care products. Both Dr. Boswell and our licensed aesthetician will discuss the different products that are available and how they would help you to improve your skin. Retinoids are effective in reducing wrinkles and can help even out the skin’s coloration, particularly when combined with skin lightening agents, such as, kojic acid or hydroquinone. Alpha hydroxy acids are mild fruit acids that help with exfoliating the skin and allowing skin care products to penetrate better. For more aggressive treatments, chemical peels (trichloroacetic acid or glycolic acid) or light based therapies (Clear & Brilliant laser, erbium micropeels, broad band light) can be utilized. For patients that are in their 30’s (occasionally late 20’s), they may start to see the benefit of injectable treatments like Botox and fillers. Patients also see significant weight shifts during pregnancy and they can sometimes benefit from fillers to restore the volume that is lost in the cheek region. Juvederm and Restylane are two of the more common hyaluronic acid fillers that are used. Lack of sleep can lead to lines around the eyes, such as, “frown lines,” “crow’s feet,” or “smile lines.” These can easily be treated with any of the wrinkle reducing neuromodulators like Botox, Dysport, or Xeomin. Breast Enhancement with Mommy Makeover The effects of pregnancy and breast feeding can be very dramatic on a patient’s breasts. The breasts become fuller due to estrogen and milk production, which causes the skin to stretch and stretch marks to form. After a woman stops breast feeding, the glandular tissue of the breast can decrease in size to a point where the breasts are less full than they were prior to pregnancy. This is called “post-partum involution” and now patients are left with two issues: skin that has been stretch out and less volume to fill up the stretched out skin creating a drooping appearance of the breasts. If there is significant ptosis (or drooping) of the breast, then a breast lift (mastopexy) can help to remove the excess skin, lift the nipple and areola back the appropriate position, and help fill out the upper part of the breast. If the patient has lost a significant amount of volume to the breast, then adding volume back in the form of a breast implant (breast augmentation) can help to correct the volume loss. Patients with post-partum involution frequently benefit from a breast augmentation after pregnancy. A common saying that our patients make about their breasts is that, “I just want back what I had prior to my pregnancy.” Some patients lose enough volume to their breasts combined with a significant amount of drooping, so that they will achieve their maximum benefit by having a breast lift and a breast augmentation, which is called an augmentation mastopexy. Finally, some patients end up with breasts that are larger (and droopier) than they were prior to pregnancy. A breast reduction will address both issues by removing skin and breast tissue to both lift the breasts and decrease the volume of the breasts. Some stretch marks are removed during a breast reduction or breast lift, but are not effective and reliable treatments for reducing stretch marks of the breasts. Tummy Tuck with Mommy Makeover The most pronounced and obvious effect of pregnancy is the expansion of a woman’s waist line. This causes the skin of the anterior abdomen to stretch and can cause separation of the rectus abdominus (“six pack”) muscles. This leads to loose skin below the belly button and frequent stretch marks of this skin along with the lower abdominal bulge caused by the separation of the rectus muscles. Diet and exercise will help with reducing the amount of abdominal fat that a patient has, but they will not help with tightening the skin, removing stretch marks, nor will it help bring the muscles back together. An abdominoplasty (tummy tuck) will remove the stretch marks, excess skin, and fat below the belly button. As part of an abdominoplasty, the rectus muscles are sutured back together along the midline to correct the separation of the muscles.

The following paragraphs will help you to learn more about an abdominoplasty (tummy tuck) procedure. What is an Abdominoplasty? Everyone wants to have a flat, toned abdomen. As a person ages, there are multiple factors that can cause you to lose that flat abdomen. Sometimes diet and exercise are all that a patient needs to redevelop the flat contour of the abdomen. However, there are some changes that are not fixable with just diet and exercise. Genetics (that spare tire that mom or dad has), pregnancy, major weight changes (up and down), previous abdominal surgery, and back issues that affect posture are some of the reasons that patients lose their flat abdomen and develop the “tummy pooch.” Separation of the abdominal muscles (rectus or “six-pack” muscles) can also worsen the abdominal bulge and this is frequently seen following pregnancies and those patients that lose a large amount of weight. It is common to have patients with normal body weight and still have an abdomen that protrudes or has sagging skin and fat. Candidates for Tummy Tuck in St. Louis For those patients that would like to have a flatter abdomen with a more toned appearance, a “tummy tuck” (abdominoplasty) may be the correct answer. An abdominoplasty typically removes the skin and fat of the lower abdomen. This allows us to visualize if there is any separation of rectus abdominus muscles (“six-pack”) muscles and then this can be corrected at the same time. Typically, the skin and fat from the about the level of the belly button down to the level of the pubic bone is removed during a tummy tuck. This also includes the stretch marks of the lower abdomen. Then the muscles are brought back closer to the midline from the breastbone to the belly button and then from the belly button to the pubic bone (think of wearing a corset inside your skin). Since the skin and fat of the upper abdomen are then pulled down to cover defect created by removing the lower abdominal skin, the entire abdomen becomes flatter, smoother, and tighter. There are several different types of tummy tucks and which one that a patient chooses depends on their anatomy, the amount of skin excess that is present, and the patient’s tolerance for an incision. A good way to estimate the length of the incision is that for every centimeter of skin that you can pinch in the vertical direction from you belly button to pubic bone will result in a transverse incision that is three times as long. This means that if you can pinch 12 centimeters, the incision will be 36 centimeters. A “mini-abdominoplasty” or “mini-tummy tuck” means that incision is shorter and that it is typically kept between the hip bones. This does limit the amount of skin that can be removed and is for patients that have a minimal skin excess. It also limits the amount plicating or tightening of the muscles that the surgeon can perform during the surgery. For a “full abdominoplasty” or “tummy tuck,” the incision extends just beyond the hip bones and allows more skin to be removed and the muscles to be tightened from the breast bone to the pubic bone. Most patients that are interested in addressing their abdomen achieve their maximal benefit from a full abdominoplasty. Patients that have lost a significant amount of weight (greater than 70 pounds), may find that incision needs to be extended all the way around their abdomen to address the loose skin of the flanks and back in addition to the abdomen. This is called a circumferential body lift or lower body lift. During your consultation, Dr. Boswell will discuss the different tummy tuck options and see which one is best for you based on your anatomy and your goals with this surgery. Since an abdominoplasty is an elective procedure, it should be performed on adults that are in good physical health and are close to their ideal body weight. It’s best if patients are within about 20 pounds of their desired weight when having a tummy tuck. If a patient is significantly larger and then loses the excess weight after surgery, then the patient can develop additional loose skin that may require more surgery. Also, the extra fat limits the amount of skin that can be removed at the initial surgery. This incision will produce a permanent scar. Just because it is plastic surgery, does not mean that it is scar-less surgery. The scar is positioned just above the pubic bone and is located low on the abdomen to try and hide the scar under a patient’s underwear or swim suit. The incision usually extends from hip bone to hip bone and the length is determined by how much tissue is removed.

Breast Augmentation Breast Lift (mastopexy) Breast Reduction Breast Reconstruction Implant Removal and Revision Male Breast Reduction (gynecomastia) Surgery of the breasts is a major focus at BodyAesthetic Plastic Surgery for several reasons. Breast augmentation is one of the most frequently-requested plastic surgery procedures. Breast reconstruction following mastectomy is another specialty we offer. Women facing breast cancer should be aware that insurance companies are now mandated to cover breast reconstruction. A breast lift, called mastopexy, is popular among women whose breasts have begun to sag after childbirth and nursing or as a result of aging and gravity. These women do not necessarily want larger breasts; they simply want to regain the more youthful position and contour of their breasts. We offer short-scar mastopexy, which produces two-thirds fewer scars than a traditional mastopexy. A breast lift can be combined with breast augmentation for women who want to get rid of breast sagging and also increase breast volume. Breast reduction is increasingly popular among both men and women. Male breast reduction to treat a condition called gynecomastia can usually be done with liposuction. For women, removal of large amounts of breast tissue is required to treat the chronic back and neck pain associated with large breasts. Breasts lose their perkiness over time due to weight changes, gravity, aging, pregnancy and nursing. As the skin loses its elasticity, it loses the ability to support the breast. Over time, volumetric changes in the breast occur causing the breasts to lose their shape and firmness. A mastopexy (or breast lift) is a procedure that will remove some of the excess skin of the breast to raise the position of the nipple and areola. This also allows the shape of the breasts to be improved, reduce the sagging of the breasts, help restore volume to the upper poles of the breasts, and correct asymmetries between the breasts. In addition, a mastopexy will reduce an enlarged areola (the dark skin around the nipple) to a more normal size. A mastopexy is a very similar procedure as a breast reduction. Both procedures reduce the size of the areolas and raise them back to a more aesthetic position on the breast. They also reduce the sagging of the breast by removing some of the excess skin of the breast. The difference is that a breast reduction also removes some volume from the breast and makes it smaller. A mastopexy doesn’t change the size of the breast. Patients that have large, pendulous breasts may benefit more from a breast reduction, whereas those patients with small, sagging breasts would do better with a mastopexy. Breasts of all sizes can be lifted; however the lifting effect will not last as long in heavier breasts. The most common reason that women proceed with a mastopexy is that pregnancy and nursing have stretched out the skin of their breasts. Another complication of pregnancy is “post-partum involution,” in which patients frequently have less volume in the breasts after they are done breast feeding than they had prior to their pregnancy. This exacerbates the sagging appearance of the breast. If a patient is planning on having additional children, then it is usually best to delay the breast lift until after they are done having children, as additional pregnancies will stretch the breast skin again. If a patient becomes pregnant after a mastopexy, they are usually able to breast feed as the milk ducts and nipples remain attached. The most noticeable disadvantage of a breast lift is that it will leave permanent scars (any incision made on the body will leave a scar). The scars improve with time and are placed so that they are covered by a bra or swimming suit. Patients who smoke tend to have more unfavorable scars than non-smokers. A short-scar mastopexy (lolli-pop shaped incision) has smaller incisions than a traditional, wise-pattern mastopexy (boat anchor shaped incision) and the short-scar mastopexy is the preferred procedure for small and medium size breasts. For patients with large pendulous breasts, the wise-pattern procedure is the better option. During the consultation, measurements of the breasts are taken and the amount of excess skin is evaluated. This allows us to determine which procedure is better for the patient. Some women would like to correct sagging of the breasts and would also like to add more volume to their breasts. These patients will typically get their best results if they have their breasts lifted and augmented. For most patients, they will have a better result if the mastopexy and breast augmentation are performed as two separate procedures, particularly if they have any significant asymmetry between the breasts. If there is a significant amount of loose skin or sagging, the mastopexy is the initial procedure. If there is a minimal amount of loose skin or sagging, then breast augmentation is the initial procedure followed by the mastopexy three to six months later. Dr. Boswell will discuss this with you at the time of your consultation so that you can make an informed decision. The Surgery Performing a mastopexy usually takes about 1.5 to 2.5 hours depending on what type of mastopexy is performed and how much excess skin that a patient has to remove. The incision for the “short-scar mastopexy” is right around the areola and then extends straight down to the inframammary crease (the fold underneath the breast). This allows the nipple and areola to be elevated to a more raised position. The resulting incision looks like a “lollipop”. In addition, the size of the areola can be reduced if it has stretched out due to pregnancy, gravity, or weight changes. The incision for the “traditional” or “wise-pattern mastopexy” is around the areola, straight down to the inframammary crease, and then extends horizontally in the inframammary crease from the sternum (breast bone) to the axilla. This incision is about 70% longer than the short-scar mastopexy and it does allow the removal of excess skin extending out to the axilla (armpit). The wise-pattern mastopexy is for the larger breasted patient with more excess skin.

Women with very large, heavy and drooping breasts may experience a variety of medical problems caused by their excessive breast weight. Symptoms typically include back, shoulder and neck pain, skin irritation and rashes beneath the breasts, stooped posture, permanent shoulder indentations caused by bra straps, and sometimes breathing difficulties. Physical activity may be restricted by the size and weight of the breasts. In addition to such physical problems, unusually large breasts can make women, or teenage girls, feel extremely self-conscious and embarrassed. Finding clothes that fit is another worry. Breast reduction, technically known as reduction mammoplasty, removes fat, glandular tissue, and skin from the breasts to make them smaller, lighter, and firmer. When needed, the surgery also reduces the size of the areola (the darker skin surrounding the nipple). The goal is to create smaller, well-shaped breasts that are in better proportion to the rest of the body. Breast reduction is usually performed to relieve physical, and often emotional, discomfort rather than simply to improve cosmetic appearance. Because of this, some health insurance policies will cover the surgery costs. The staff at BodyAesthetic Plastic Surgery will help determine if surgery might be covered by insurance. In most cases, breast reduction is performed after the breasts are fully developed, but it can be done earlier if large breasts are causing serious physical discomfort and/or psychological difficulties for teenagers. The best candidates for reduction are those who are mature enough to fully understand the procedure and have realistic expectations about the results. Breast reduction is usually not recommended for women who intend to breastfeed in the future because the surgery removes some of the glandular breast tissue, which can decrease milk production. Breast reduction is not a simple operation, but it is safely performed by trained plastic surgeons. The procedure does leave noticeable and permanent scars, though they will be covered by a bra or bathing suit. (Poor healing and wider scars are more common in smokers and dark-skinned individuals.) The procedure can also produce breasts that are slightly asymmetrical or unevenly positioned nipples. However, most women with very large breasts are not symmetrical to begin with. Some patients may experience a permanent loss of feeling in their nipples or breasts. In a very small percentage of women, the nipple and areola may lose their blood supply and the tissue will die. In such rare cases, the nipple and areola can be reconstructed with skin grafts taken from elsewhere on the body. Breast Reduction: The Surgery Breast reduction surgery is performed in the hospital on either an outpatient or inpatient basis, depending on whether you need to stay overnight in the hospital after surgery. The length of the procedure is determined by the technique used and amount of tissue removed but usually takes 2 to 4 hours. Techniques for breast reduction vary and depend on the size of the breasts and the degree of reduction the patient wants. The procedure we use most often includes an incision that circles the areola and then extends downward vertically to the inframammary crease (the natural fold below the breast). This short-scar breast reduction leaves a scar shaped like a lollipop. If the breasts are very large, another incision that runs horizontally along the inframammary crease may be needed. This traditional reduction mammoplasty scar is shaped much like a boat anchor. After the excess glandular tissue, fat, and skin are removed, the nipple and areola are moved into their new, higher position. The remaining skin and tissue are then draped around the areola until a pleasing breast shape and contour are achieved. Liposuction is sometimes used to remove excess fat from the area near the armpit. In most cases, the nipples remain attached to their blood vessels and nerves. However, if the breasts are very large or pendulous, the nipples and areolas may have to be completely removed and grafted into a higher position. This will result in a loss of sensation in the nipple and areolar tissue, and breastfeeding will be impossible. What To Expect After Breast Reduction After surgery, the breasts are wrapped in an elastic bandage or a surgical bra placed over gauze dressings. This dressing is usually removed 4 to 5 days after surgery and replaced with a soft sports bra that provides support and is worn 24 hours a day for several weeks, until the swelling and bruising subside. Most stitches are absorbable and do not have to be removed.

Rejuvenation of one or more areas of the face is among the procedures most often requested by BodyAesthetic patients. These range from a full facelift to the newer techniques of short-scar, mini, and midface lifts, which require specialized training. Another procedure that demands advanced training is the endoscopic browlift, which uses four very small incisions to lift the brow and smooth the forehead. An eyelid lift (blepharoplasty) is appropriate for patients whose lids have begun to sag with age and tend to produce a “tired” appearance. Other procedures address skin resurfacing: lasers, dermabrasion, and deep chemical peels all bring fresh skin cells to the surface and thereby minimize wrinkles and give skin a more youthful appearance. The re-contouring of specific features is also in high demand. Some people have never been happy with the shape of their nose or ears, and some request neck rejuvenation to get rid of excess fat or sagging skin that makes them look older than they are. The Benefits of Facelift As people age, the effects of gravity, exposure to the sun, and the stresses of daily life can be seen in their faces. Deep creases form between the nose and mouth; deep wrinkles are prominent, especially in the cheeks, and the skin sags; the jaw line grows slack and is less well-defined; skin folds and fat deposits appear around the neck. A facelift (technically known as rhytidectomy) improves the most visible signs of aging by removing or repositioning subcutaneous tissue, excess fat, tightening underlying muscles, and redraping the skin of your face and neck. A facelift can be done alone or along with other procedures such as a forehead lift, eyelid surgery, or nose reshaping. The result of a facelift is a younger and refreshed look. The best candidates for a facelift are men and women whose face and neck have begun to droop but whose skin still has good elasticity and well-defined bone structure. Most patients are between the ages of 40 to 70, but facelifts can be done successfully on people in their 70s or 80s. Facelifts are very individualized procedures because no two faces are exactly alike. In your initial consultation, your surgeon will evaluate your face, including the skin and underlying bone structure, and the severity of wrinkles and folds. He will also discuss the benefits of your facelift at our St. Louis plastic surgery facility, and ask about your goals for the procedure. Facelift : The Surgery Incisions usually begin above the hairline at the temples, extend in a natural line in front of the base of the ear to just inside the cartilage at the front of the ear, and continue in the crease behind the ear and into the lower scalp. If the neck needs lifting, a small incision may also be made under the chin. In general, the surgeon separates the skin from the fat and muscle below. Fat may be trimmed or suctioned from around the neck and chin to improve the contour. The surgeon then tightens the underlying muscle and fascial tissue and elevates the skin upward and back, then trims off the excess. The stitches used to secure the layers of tissue and close the incisions are typically removed after 5 to 7 days. Because your scalp may take longer to heal, the stitches in your hairline may be left in a few days longer. Your head will be wrapped loosely in bandages after surgery to minimize bruising and swelling, but they will be removed the following day, as will two small drains placed during surgery. When the bandages and drains are removed, you may shower and wash your hair. Although your face will be pale, bruised, and puffy at first, you’ll look much better in a week or so. If you need it, special camouflage makeup can mask most bruising that remains. What To Expect After A Facelift There usually isn’t significant discomfort after surgery, but your surgeon will give you a prescription for pain medication. (Severe or persistent pain or sudden swelling of the face should be reported to your surgeon immediately.) Some numbness of the skin is quite normal, but it will disappear in 3 to 6 months. Your head should remain elevated and as still as possible for a couple of days after surgery to reduce swelling. Once you’re up and about, take it easy for the first week after surgery. Be especially gentle with your face and hair, since your skin will be both tender and numb. Your surgeon will give you specific guidelines for gradually resuming normal activities. About 85% of the swelling will be gone in about 3 weeks, but the final results of a facelift may not be fully apparent for up to 6 months. Initially, your face will probably look and feel strange, with your features distorted from the swelling and facial movements slightly stiff. The scars from a facelift are usually hidden by hair or in the natural creases of the face and ears, but they will fade over time and should be scarcely visible. Results of a facelift are not permanent and you’ll continue to age with time. However, the effects are lasting in the sense that you have “turned back the clock.” Seven to ten years after a facelift, you’ll look basically the way you did before surgery. An eyelid lift (called blepharoplasty) removes or repositions excess fat, skin, and muscle from the eyelids. It can correct drooping of the upper eyelids (called an upper blepharoplasty) and/or the puffy bags below the eyes (called a lower blepharoplasty). These eyelid problems make people look older and chronically tired and may even interfere with vision. An eyelid lift won’t remove crow’s feet or other wrinkles, eliminate dark circles under the eyes, or lift sagging eyebrows. (Other procedures are required for treating these problems.) While blepharoplasty can add an upper eyelid crease to Asian eyes, it will not erase evidence of your ethnic or racial heritage. Sagging in the upper cheek area is not uncommon as we age, and a cheek lift is typically done at the same time as an eyelid lift. Basically, a cheek lift is an extended lower blepharoplasty. An eyelid lift can be done alone or in combination with other facial procedures such as a facelift or browlift. Most patients requesting an eyelid lift are 35 or older, but droopy upper eyelids or baggy lower lids sometimes run in families. Patients with hereditary drooping or bags may decide to have eyelid surgery at a younger age. A few medical conditions make blepharoplasty more risky, including thyroid problems (such as hypothyroidism and Graves’ disease), dry eyes or lack of sufficient tears, proptosis (“bug eyes”), high blood pressure or other circulatory disorders, cardiovascular disease, and diabetes. A detached retina or glaucoma is also reason for caution; if you have these problems, check with an ophthalmologist to see if a blepharoplasty is safe. The Surgery The surgeon makes incisions along the natural lines of your eyelids: in the creases of your upper lids and just below the lashes on the lower lids. The incisions may extend into the crow’s feet or laugh lines at the outer corners of the eyes. If the cheeks are to be lifted, the lower lid incisions are modified to access the cheek tissue in an extended lower blepharoplasty. Working through the incisions, the surgeon separates the skin from underlying fatty tissue and muscle, removes or repositions excess fat, and often trims sagging skin and muscle. The incisions are closed with very fine sutures, which are removed 2 to 7 days after surgery. Once the stitches are out, the swelling and discoloration around the eyes gradually subsides. If you have a pocket of fat beneath your lower eyelids but don’t need to have any skin removed, your surgeon may perform a transconjunctival blepharoplasty, which is usually done on younger patients with bulging fat deposits under the eyes but no skin excess. In this procedure the incision is made inside the lower eyelid and therefore leaves no visible scar. After surgery, the eyes are lubricated with ointment and a bandage applied. Your eyelids may feel tight and sore as the anesthesia wears off, but you can control any discomfort with the pain medication prescribed by your surgeon. If you feel any severe pain, call your surgeon immediately. What To Expect After An Eyelid Lift You should keep your head elevated for several days and use cold compresses to reduce swelling and bruising. Bruising varies from person to person but usually reaches its peak during the first week and gradually subsides between 2 to 4 weeks. Keep activities to a minimum for 3 to 5 days and do not engage in strenuous activity for about 2 weeks. It’s especially important to avoid activities that raise your blood pressure, including bending, lifting, and rigorous sports. You should be able to read or watch television after 2 or 3 days. However, you won’t be able to wear contact lenses for about 2 weeks, and even then they may feel uncomfortable for a while. You’ll be shown how to cleanse your eyes, which may be gummy for a week or so. Since your eyelids may feel dry at first and your eyes may burn or itch, eye drops are recommended to ease the discomfort. For the first few weeks you may experience excessive tearing, sensitivity to light, and temporary changes in your eyesight, such as blurring or double vision. Your surgeon will follow your progress very closely for the first week or two. Most people feel ready to go out in public (and back to work) in 7 to 10 days. By then, depending on your rate of healing, you’ll probably be able to wear makeup to hide the bruising that remains. You may be sensitive to sunlight, wind, and other irritants for several weeks so wear sunglasses and a special sunblock made for eyelids when you go out. Healing is a gradual process, and your scars may remain slightly pink for six months or more after surgery. Eventually, though, they’ll fade to a thin, nearly invisible white line. A browlift (also called a forehead lift) corrects the drooping or low position of the eyebrows that gives the face a tired or sad appearance. It also softens horizontal creases across the forehead, as well as the vertical lines (frown lines) between the eyebrows that contribute to an unhappy or angry expression. Many people have eyebrows that are not positioned symmetrically, and this can be corrected with a browlift. A lot of patients who request a forehead lift say that people ask if they are mad, tired, or worried because they always seem to be frowning. A browlift rejuvenates the area between the upper eyelids and the scalp across the entire forehead (temple to temple) to produce a more relaxed and refreshed appearance. It is most often done on people over 40. Many patients elect to have a browlift in combination with other facial procedures, such as a facelift or eyelid lift (blepharoplasty). The Surgery Dr. Boswell is trained to perform a browlift with an endoscope, which requires 4 or 5 short incisions in the hair. The endoscopic approach is not as successful as the traditional browlift for treating horizontal creases across the forehead, but it does effectively raise the eyebrows and improves the vertical frown lines at the top of the nose. The scars from an endoscopic browlift are usually imperceptible, and the healing time is shorter than with a traditional browlift. If the amount of forehead skin is excessive, or if significant horizontal wrinkles are present, a traditional browlift may be the best option. In this case, an incision is made across the top of the scalp, just behind the hairline. In patients with a high forehead, the incision is placed just below the hairline so the hairline will not be raised to an unnaturally high position. The resulting scar usually blends nicely into the hairline. Whichever incision is used, a browlift removes excess forehead tissue and skin, repositions the muscles and tissues that cause wrinkling and frown lines, and tightens the forehead skin. The result is that the eyebrows are raised and the forehead smoothed. The procedure takes about 1 1/2 hours and may be done on an outpatient basis. If a browlift is combined with other procedures, such as a facelift or eyelid lift, patients usually spend one night in the hospital. What To Expect After A Browlift A light dressing is placed over the incision, but it is removed the day after surgery, at which point you may shower. Your scalp will be tender and you’ll have some discomfort for a few days, but your surgeon will prescribe medication that should relieve most of the pain. For the first day after a browlift, your head should remain elevated. Even with this precaution, swelling and bruising of the forehead will increase over the first 2 to 3 days before it begins to subside. Because the skin and tissues surrounding the eyes are thin, some swelling and bruising usually occurs around the eyes, as well. Stitches are removed 7 to 10 days after surgery, and you should feel and look good enough to return to work – or in less time with an endoscopic browlift. By about 10 days postop, most of the bruising should be gone, and what remains can be concealed with make-up. By this time, you will definitely notice that your facial expression seems more refreshed and relaxed. Within 3 weeks, about 85% of the swelling has subsided. However, your final appearance may not be fully evident for up to 6 months. Both treatment types reduce the appearance of facial lines and wrinkles, but they work in different ways. Relaxers such as Dysport and Botox injections can minimize the appearance of facial lines while also helping to prevent the development of new creases. Tissue filler injections fill in facial lines that have already appeared. Tissue fillers do what the name implies: they plump up facial lines and creases that are too deep to be improved with treatments such as microdermabrasion or chemical peels (available in our Skincare Center). Injections only performed by Dr. Craig Boswell who is a National Trainer for Botox and an Expert Injector awarded by Allergan. No facial rejuvenation product has gained faster popularity among both men and women than Botox, or botulinum toxin. The demand for Botox directly relates to its safety record and proven ability to reduce and prevent certain facial lines and wrinkles. Dynamic facial lines are caused by the repeated contraction of muscles when we smile or frown. As we age, our skin becomes less elastic and therefore less able to spring back after a muscle contraction. Over time, the natural expression lines in the brow and around the eyes become more apparent, and more permanent. Injections of Botox Cosmetic block the nerve impulses that contract certain facial muscles. When these muscles stay relaxed, crow’s feet around the eyes, vertical lines between the brows, and horizontal forehead lines smooth out because their cause is eliminated. If you are considering Botox, be aware that results are not immediate. Improvement in facial lines doesn’t occur for about a week, and full effectiveness takes 2 weeks. There is no need to worry about seeming expressionless after Botox treatment. You will still look perfectly normal when you laugh, smile, or frown. Botox injections continue working for about 4 months. Patients who have used Dysport (another botulinum toxin) may use Botox Cosmetic, but the two products are not the same. The units of Botox and units of Dysport per treatment are not interchangeable. In clinical trials for FDA approval, the patients treated with Botox received 20 units and the patients treated with Dysport received 50 units. Dysport does seem to spread or migrate farther than Botox Cosmetic which means that different patterns for injection need to be used for the two botulinum toxins. When injections are performed around the mouth for perioral wrinkles or to treat the down turned corners of the mouth, Botox Cosmetic is probably a better product because it is more precise and has less migration. Dysport may have a slightly faster onset of action. Dysport and Botox Cosmetic have similar longevities, meaning they both last for about the same length of time. Excessive Perspiration (Hyperhidrosis) In addition to its use for reducing facial lines in the forehead and around the eyes, Botox is extremely effective in treating hyperhidrosis, or excessive perspiration. Embarrassing sweating of the forehead, armpits, palms of the hands, or soles of the feet is a common problem with no permanent solution, but Botox injections basically stop the sweating. When used for hyperhidrosis, Botox effectively reduces perspiration for approximately 1 year. People who seek hyperhidrosis treatment are often involved in careers that require being in close proximity with clients who might be offended by underarm odor. Businessmen and women who tend to develop sweaty palms and must shake hands frequently are also grateful for Botox treatments.

Perhaps the most effective and long-lasting filler is a patient’s own fat transferred from another part of the body to fill lines or sunken areas in the face. Some patients undergoing liposuction or another type of body contouring choose to have some of the fat transferred to facial areas.
